The following are forms that may be usefull for you troop.

• New Medical for 2010
  • The National Boy Scouts of America has released this new Medical form. It has replaced the Class 1&2 and Class 3 forms and is now the ONLY medical form accepted at Camp. It can be used for any Scouting activity from Summer Camp to National Jamboree. The BSA Requires this form be used by everyone starting in 2010. It is only good for 12 months. • Camp Staff Application
  • Intersted in becoming a member of the Camp Chawanakee Staff? We're always looking for scouts who are willing to help make Camp Chawanakee a premier camp for scouts.
• Counselor In Training Application
  • Join the Camp staff at the age of 14-15 with this application to become a Councilor in Training.
• Camp Map
  • A scout is prepared! Be sure to know where everything is at camp with this handy map.
• Tour Permit
  • Required by National for all troops that go on campouts this form must be filed with your local council at least two weeks before you attend camp. This version of the PDF also allows you to fill it out on your computer and print it out.
• Consent for Minor Form
  • Necessary for youth (Scout age 17 and below) who is attending camp. Be sure to have a copy with your Medical Form.
